Arizona Department of Health Services (ADHS) Assisted Living Facility License

Required for all assisted living operations in Arizona. Covers resident care standards, staffing requirements, and facility safety compliance.

Arizona Behavioral Health Facility License

Covers behavioral health services provided within our second chance and veteran housing programs.

Arizona Department of Economic Security (DES) Certification

Certification for providing services to individuals referred through state agencies and programs.
